Current patients should contact their provider through Spruce or the patient portal. Alternatively, you may call the office at (563) 412-4177. Calls will be returned within one business day.

New patients should request a consultation by submitting the online screening form. If there are any questions, please call (563) 412-4177. 

Once you have submitted the online screening form and been accepted as a new patient, you will receive an email inviting you to the patient portal. After creating an account, you will receive the new patient welcome packet and forms to complete before your first appointment. You can use the portal to communicate with your provider and update your information.  

Providers interested in referring a patient should ask the client to request a consultation through the process outlined above. New patient requests do not require a referral.
