These forms are available through the portal under “My Documents”. They are also located here in case you have trouble accessing the portal. 

The CAF policies form can be reviewed and electronically signed in the portal and does not need to be printed.

Please print and complete both the Release of Information and Credit Card on File forms. Upload the completed forms to the portal as an attachment or bring them with you to your first appointment.
